EMS | Medical Questionnaires
Description
Medical Questionnaires provides the ability to group Medical Types and present these in the form of a questionnaire or consultation form.
Resolution
Creating Medical Questionnaires
In "Administrator" click onto "Initial Setup Menu" 
then click onto "Medical Types" or via "Configuration" click onto “Medical Types”
- To create new Medical Questionnaire click onto the "Green Plus"
- Enter a description
- Move the required Sites from left to right to allocate
- Select the "Green Tick" to confirm
Configuring Questionnaire
The questionnaire can be configured at site level with the question to be included and a header and footer.
Alternate Description - ability to change the description to meet the sites business requirements
Header - use the "Tick Box" to add a header to the questionnaire, Select "View/Edit Description" to add text and images
Footer - use the "Tick Box" to add a footer to the questionnaire, Select "View/Edit Description" to add text and images
Include
General Note - activates the ability to enter notes at the end of the questionnaireStaff Name - adds a drop down to select the name of the staff member which assisted with the completion of the questionnaireStaff Signature - requires the staff member to counter sign the questionnaire to indicate they have read the answers
Allocate - Move the required Medical types from left to right to be included in the questionnaire
