Premier | Medical Questionnaires

Description

Medical Questionnaires provides the ability to group Medical Types and present these in the form of a questionnaire or consultation form.

Resolution

Creating Medical Questionnaires

In "Administrator" click onto "Initial Setup Menu"  then click onto "Medical Types" or via "Configuration" click onto “Medical Types”

1. To create new Medical Questionnaire click onto the "Green Plus"
   
2. Enter a description 
   
3. Move the required Sites from left to right to allocate
4. Select the "Green Tick" to confirm
   

Configuring Questionnaire

The questionnaire can be configured at site level with the question to be included and a header and footer.

Alternate Description - ability to change the description to meet the sites business requirements

Header - use the "Tick Box" to add a header to the questionnaire, Select "View/Edit Description" to add text and images

Footer - use the "Tick Box" to add a footer to the questionnaire, Select "View/Edit Description" to add text and images

Include

General Note - activates the ability to enter notes at the end of the questionnaire

Staff Name - adds a drop down to select the name of the staff member which assisted with the completion of the questionnaire

Staff Signature - requires the staff member to counter sign the questionnaire to indicate they have read the answers

Allocate - Move the required Medical types from left to right to be included in the questionnaire
